Office hours are Monday through Friday from 8:30 a.m. to 5:30 p.m. A pre-recorded message will instruct you when the office is closed.
Call 734-7850. To enable a better selection of time and date, please call for appointments as early as possible. If you are unable to keep an appointment, please notify us 24 hours in advance. This will provide the opportunity for another patient in need of care to be seen.
Emergencies: If you need to consult with a doctor after regular office hours, you may call the Cullman Regional Medical Center operator at (256) 737-2000. The operator will assist you in reaching a doctor.
We appreciate and encourage insurance co-pay and deductible payments for services at the time of your visit. If you have any questions regarding our fees, our staff is available to assist you with financial arrangements. For your convenience, we accept credit and debit cards. We can assist you as well if you do not have insurance coverage and need financial arrangements for services provided.
As a service to you, our office will prepare and submit all insurance claims for you. However, we do consider the patient primarily responsible for the account. Please present your insurance identification card at the time of your visit. You will also be ask to present you driver's license at the time of the visit for additional information for verification.
We are participating Medicare physicians. All claims will be filed for you. You will be responsible for your annual deductible, co-pay (if applicable) and the 20% Medicare does not pay. If you have Medicare supplement insurance, we will file a claim to them for the 20% balance as a courtesy to you.
We also accept most all other Insurance plans. All claims will be filed for you. You will be responsible for your annual deductible, co-pay (if applicable) and any remaining percentage that the insurance does not pay that is the patient's responsibility. We ask that you pay your responsibility amounts at the time of service.
If you do not have insurance you will need to make financial arrangements with our staff prior to services being provided.
